Muskogee Health Department will be at Ben Franklin for an adolescent health clinic on Wednesday from 11:30 to 2:30

Oklahoma State Department of Health Mobile Adolescent Clinic visiting MPS!

  • Wednesday, November 1st, 2023 at Ben Franklin 6th & 7th Grade Academy, 11:30-2:30
  • Wednesday, November 29th, 2023 at Alice Roberts 8th & 9th Grade Academy- 9:00-2:00
  • Thursday, November 30th, 2023 Muskogee High School- 9:00-2:00


Services Provided: 

1) Nurse Practitioner available for advanced testing such as strep, flu, etc.; Physical exams; Athletic Physicals

2) School-required and recommended vaccines available for children that meet the guidelines: uninsured, non-insured, or Native American/Native Alaskan (no CDIB card required); Children that have private insurance, will have flu vaccines available.


**Required consent forms must be filled out prior to the visit by the parent/legal guardian

  • The initial history is required to be completed once for all child and adolescent clients. 
  • There are two consent forms, one is for well and/or sick child visits and the other is for sports physical only. 
  • Non-OSDH permission form attached, this must be completed and signed by the legal guardian on the date that services are to be received. Any permission slip must include the legal guardian's name, the child's name, who is being granted permission to seek care for the child, the date of actual service, and the legal guardian's signature.
  • The Release of Information form is to be completed if the legal guardian wishes for any information to be released to the school district. 
  • The Screening Checklist for Vaccine administration is to be completed by the legal guardian as needed.
