The requirements to be eligible for the Title VI Program:
* Either the child, their parent, or their grandparent must be an enrolled member of a Federally recognized, State recognized, terminated tribe, or organized Indian group.
* The enrollment number establishing the membership of the child, parent, or grandparent, or other evidence of membership (i.e. affidavit from tribe, CDIB card or birth certificate). The name and address of the organization that maintains updated and accurate membership data for such tribe or band of Indians must also be provided.
The requirements to be eligible for the JOM Program:
* Child is a member of a Federally recognized tribe with a CDIB or tribal membership card with enrollment number
OR
*Parent is a member of a Federally recognized tribe with a CDIB or tribal membership card enrollment number, and is half or more blood quantum. Parent must bring their card proving blood quantum and a state certified birth certificate for the child with the parent's name on it.
